So what happens? Is it their insurance that covers it or yours? I feel like all they do when I test drive is just get ahold of my driver's license.

In the US, the car carries the insurance, not the driver. So be mindful of who you let drive your car. That said... the car's insurance company can go after the driver's insurance company for the claim. The most you will likely be out is your dedectible, but can count as a claim against you.
