Upon receipt of your approved affidavit, you will be issued a tire award certificate entitling you to any set of Yokohama tires up to a maximum size of 19” in diameter. Please know that you will need to contact Yokohama Tire at the phone number on the certificate by October 29, 2010 in order to claim your prize, or the prize will be forfeited. Please see the Official Rules for further prize details and eligibility requirements. This prize has an approximate retail value of up to $2,604.

In order to confirm your win, the Affidavit of Eligibility, Liability & Publicity Release (“affidavit”) must be completed.
