Check bomb bay
Empty - fly plane
Loaded with Nukes - get another plane

the checks were done, however teh system that does checks and balances for where the nukes are, was messed up and they loaded them un known to anyone. well, the pilots checked the one side and said, yup, all good... and bam, missed em
