Really? That's it? I know those things off the top of my head. I figured the card was absolutely needed. First time with insurance, so I'm still learning how the game is played.

yah, they will call your provider with your ss number and be able to get the account. now if your doc's place is a pos or your provider, then the store will tell you to come back later for the card, or ask you to call later with the account number.

ive been going to eye docs since the late 70's so i knowz these things
