Yes, or more precisely, the manifold vacuum's negative pressure sucks air from the crankcase via these lines

The other pair of tubes that go to the sides of the intake tubing feed fresh air from the intake to the crankcase.

The V-shaped nippes on the front of the manifold. The manifold vacuum that sucks crankcase gases into here is generated by your engine's desire to draw intake air. So all manifold vacuum inlets are effectively engine-breathing-air inlets. Anything you stuff in them gets burned by the engine eventually.

Yes. Ideally a can with some sort of baffle/mesh system. The V-shaped ports in the center would be the clean side(s) of the can(s), then other end is what you want the baffles/mesh to trap stuff from.
