You have been selected as the winner of a set of four (4) Yokohama Tires (up to 19"). Upon receipt of your approved affidavit, you will be issued a tire award certificate entitling you to any set of Yokohama tires up to a maximum size of 19” in diameter. Please know that you will need to contact Yokohama Tire at the phone number on the certificate by October 29, 2010 in order to claim your prize, or the prize will be forfeited. Please see the Official Rules for further prize details and eligibility requirements. This prize has an approximate retail value of up to $2,604.

In order to confirm your win, the Affidavit of Eligibility, Liability & Publicity Release (“affidavit”) must be completed. Please fully complete and notarize the affidavit and return your original copy. Your affidavit must be received within five (5) calendar days of the date of your initial win notification. If you do not return your affidavit within this timeframe, another winner will be selected. You can find the mailing address at the top of the affidavit.
You will be notified of your approval status once we have received and reviewed your fully executed Affidavit of Eligibility, Liability & Publicity Release (“affidavit”). Prize fulfillment will begin within 8-10 weeks after the conclusion of the Sweepstakes..
