Yes the bumper has to come off, and no it is not twice a year.
K&N filters are good for 50,000 miles before a recharge is needed and then another 50,000 when you replace them or recharge them again. Recharge is cleaning/re-oiling them.

Also, bumper removal is around 10-15 minutes after the first time and you learn where all the clips & bolts are. We have taken mine off twice now and it is extremely easy, hardest part is getting the bolt in the wheel well area but that is a breeze now since we know where to look.
