Congratulations on being able to make it to that high of mileage. Toyota and Lexus vehicle reliability and dependability is hard to beat with Nissan and Infiniti being not too far behind them from what I have seen. Doing your general maintenance go’s a long way really. Check your valve gaps and what the clearance is to find out if it is with in Toyota spec? If you have to at that mileage I would purchase brand new fuel injectors if you have to. From the pics engine does not look too bad doing oil changes on time helps a lot. The 2003 Toyota Camry we had ex taxi made it to 402K miles before it was traded in and it still ran fine. Toyota’s are great vehicles all around.
