I've seen chit like that happen a lot of times. Had one guy with long hair in a pony tail get scalped. Hair got caught in the lathe. Pulled the scalp right off the top of his head. No more long hair. Seen the aftermath pictures of a worker getting his safety harness caught in the 30" diameter coupling of a steam turbine/generator. His harness got caught and drug him in and spun him around at 3600 rpm for an hour before they could shut it down.
