With the metal sleeve removed you can reinstall the bolt and screw it in until it hits the wheel hub. Keep tightening it and the leverage will pop the rotor off no problem. The rear rotors don't have the bolt installed, but I'm pretty sure they have a threaded hole that you can use the bolt in same as the fronts.
Oh man! That's amazing, thanks Jar. I had no idea that's what those were for. Explains a ton. I knew I had to be missing something, I was really smacking the rotors pretty hard before I could get them to loosen.
