Also it could be from an unsupported case head in the barrel. Look at your spent casings to see if there is a bugle on the case, just above the case head. Some Glocks and S&W M&P Shield 9mm's will bugle the cases. Some so bad that you can't reuse the case for reloading.

My 92FS gets a steady diet of 9mm Plus P's.
