Default Forced Induction motors that have died

After reading all of the recent stories of motors that didn't survive going F.I. I'm curious about how often this has happened. If you have had a motor die on you I would like to know the details of what happened.

What was the WHP?
What wtrq?
How long did it run before it died?
Was it ever 100% running right before it died?
What kit?
What PSI?
What was the cause if you know?
What year was the car?
