Does anyone know if this is a good price or above average or average? Or is this guy trying to rip me off because the car dealership is trying to reel me in using their "100% ENGINE LIFE WARRANTY!" ?

Personally any dealer only warranty is sketchy at best. Follow the Factory maintenance. Radiator conditioner is heaping pile of crap for example.
15K should be oil and air filters (2 engine 1 cabin). 175-200 sounds about right given part cost and labor for what I list (not the lesser tasks you listed)
