There's specific things you can hear... like how when he hits sustained higher notes, you can hear him struggle (like, when you talk and you're sick, have something in your throat).
It's not as outlandish as people think, I can't exactly sing in a falsetto but I just take my apparently "baritone" voice, pull some muscles in my neck, next thing you know I sound like a 5 year old girl... I know, it's scary.
